How do you create a logo box on Youtube...?

Sometimes people on YT have a logo box above their video(on the video page) that you click to go their channel. And some have banners at the top of their channels that displays Facebook and Twitter icons. If you click one of them, it takes you to their Facebook or Twitter page. How do you do both of these things???
